Patrick Rask, MD

Medical Director

Dr. Rask founded Corvallis Pain and Spine in 2011, after completing his subspecialty fellowship in interventional and comprehensive pain medicine at the Pacific Pain Treatment Center in San Francisco. His mission is to restore function and enhance the quality of life for individuals with chronic pain through advanced procedural and minimally invasive surgeries.

Patrick Rask, MD

Dr. Rask is BLS and ACLS Certified as well as fellowship trained in Interventional Pain Management. His professional memberships include the OMA, American Academy of Pain Medicine, North American Neuromodulation Society, International Association for the Study of Pain, Pain Society of Oregon, and the American Society of Regional Anesthesia and Pain Medicine. Dr. Rask performs procedures at Waverly Lake Surgery Center in Albany, OR as well as at both the Corvallis Clinic Surgery Center and Good Samaritan Regional Medical Center in Corvallis, OR. In his spare time, Dr. Rask enjoys spending time with his family, playing guitar, biking, fishing, and hiking.

David Frome, MD

Provider

Dr. Frome completed his anesthesiology residency at the University of Arizona, followed by a fellowship in interventional and comprehensive pain medicine at Loma Linda University. His mission is to identify the root cause of pain and address it using advanced procedural techniques, implants, and minimally invasive surgical procedures.

David Frome, MD

Dr. Frome grew up an Oregon native in the Mid-Willamette Valley. He attended medical school at Northwestern University Feinberg School of Medicine in Chicago, Illinois. During medical school, he conducted research and published his findings in several areas of medicine including sports medicine and scoliosis. He completed his internship in Internal Medicine as well as his residency in Anesthesia at the University of Arizona, which included a robust training in ultrasound guided nerve-blocks. He completed a subspecialty training fellowship in Chronic Pain Medicine at Loma Linda University. In his spare time, he enjoys hiking, and boating.

Jordan Garman, DO

Provider

Dr. Garman is dual Board certified in physical medicine and rehabilitation, as well as interventional and comprehensive pain medicine. He completed his subspecialty fellowship at the University of Alabama at Birmingham. He is deeply committed to reducing chronic pain and restoring individuals’ quality of life.

Jarod Z. Smith, DNP

Provider

Jarod Smith has been with Corvallis Pain and Spine for more than a decade. During this time, he completed his Master of Science in Nursing (MSN) at South University in Florida and earned his Doctor of Nursing Practice (DNP) at Frontier Nursing University. He is passionate about helping patients achieve their personal goals for improvement in their chronic pain states.

Jarod Z. Smith

Jarod is a family nurse practitioner. He completed his MSN at at South University in Tampa, FL and his Doctorate of the Nursing Practice from Frontier Nursing University. He has been practicing chronic pain management for over a decade.


Dr. Smith received his Associates in Nursing from State College of Florida and went on to receive his Baccalaureate and Masters degrees from South University in Tampa, Florida. In June of 2019, Dr. Smith completed his Doctorate in Nursing Practice from Frontier Nursing University. He is BLS, ACLS, and PALS certified. Dr. Smith is a certified nurse practitioner through the AANP. Dr. Smith approaches patients with a personalized multidisciplinary approach to reach target outcomes. He is married with three children. In his spare time, he enjoys fencing and reading.

C. Jason Smith, PA-C

Jason Smith has worked as a physician associate for more than a decade with Corvallis Pain and Spine. He graduated with honors from South University in Florida. His mission is to explore both the psychosocial and musculoskeletal aspects that affect individuals with chronic pain, aiming to address core issues that can lead to improvements in both chronic pain management and overall quality of life.

C. Jason Smith, PA-C

Jason was born and raised in Florida. He graduated summa cum laude from the University of South Florida in 2010 and received his Master of Science Degree in Physician Assistant Studies in 2013 from South University. He is a member of the American Academy of Physician Assistants and BLS certified. He loves living in Oregon with his wife and pets. In his free time, he enjoys reading and fencing
